Presentation Title:

An epidemiological analysis of Guillain-Barré Syndrome in the neurology service of the UNIMED BH Hospital

Abstract

Guillain-Barré Syndrome (GBS) is a rare immune-mediated condition that affects the peripheral nervous system and is characterized by an ascending flaccid paralysis, however, it sometimes presents in an atypical form with clinical variants. It has a diagnosis based on clinical, CSF and electrophysiological criteria. In this series of cases we portray patients admitted to the Neurology service of the Unimed Hospital of the Contorno Unit (Belo Horizonte/MG) with a diagnosis of Guillain-Barré Syndrome between April 2022 and June 2024. 29 patients were included, 66% male and 34% female. The average age was 40.3 years (range 15 - 77 years). Of these, 27% had a respiratory or gastrointestinal infection preceding the condition; 38% had been vaccinated in the last 4 weeks and 80% of patients who had an infectious trigger in the gastrointestinal tract required transfer to the ICU. The months with the highest incidence of cases were December 2022 and November 2023. The majority received treatment with human intravenous immunoglobulin (86%) and none received treatment with plasmapheresis. Half had an Erasmus GBS Respiratory Insufficiency Score (EGRIS) ≤ 3 points and the other half EGRIS ≥ 4. Three patients developed respiratory failure in the first week after the onset of symptoms. Of those with EGRIS ≤ 3, 30% went to the ICU and did not require invasive ventilation. In relation to those with EGRIS ≥ 4, 70% went to the ICU, and of these 28% required invasive ventilation. There was 1 death. Our data and results presented are compatible with those described in the literature regarding triggers for autoimmunity and prognoses. We question whether the coronavirus pandemic could be an aggravating factor for the increase in the incidence of Guillain Barré Syndrome.
