Pain as narrative: Symbolic expression of trauma and the multidisciplinary treatment of chronic pain

Abstract

This paper explores the concept of pain as a narrative form and symbolic expression of unresolved trauma. Two clinical cases are presented: a 26-year-old male with fibromyalgia and neuropathic pain, and a 43-year-old woman with chronic interstitial cystitis. In both cases, physical symptoms are interpreted as "somatic narratives" of deep emotional suffering, linked to adverse childhood experiences and familial conflict. 

The first case shows how fibromyalgic and neuropathic pain can serve as a defense mechanism and sensory memory of trauma, preventing violent impulses and providing psychic containment. The second illustrates how interstitial cystitis reflects psychological encapsulation of grief and guilt, impeding emotional flow and symbolizing unresolved inner conflict. 

These cases highlight the body’s function as a narrator of pain when verbal expression is inhibited. An integrated therapeutic approach—psychiatric, psychotherapeutic, and medical—enabled patients to access and re-signify the emotional roots of their pain, reducing the need for pharmacological treatment. 

This multidisciplinary model positions pain not only as a medical symptom but as a complex, meaningful narrative requiring a therapeutic space where body and psyche can be jointly addressed.
